This form will setup TOTP (time-based one-time passwords) and two-factor authentication for all of your accounts. (If you have multiple accounts, you only need to setup TOTP for one account and it will apply to all of your accounts.)
This service requires an Android or iPhone/iPod Touch/iPad device with a TOTP-compatible app, such as Google Authenticator, Microsoft Authenticator, or Duo Mobile. For help installing Google Authenticator, please visit the Google Authenticator help page. An account with any other service is not required. It is not necessary to enable 2-step verification for your Google/Microsoft/other account.
To setup TOTP, you must first choose a personal identification code (PIC). The PIC is case-sensitive and is made up of at least 4 numbers and/or letters. No special characters may be used. Do not use your bank PIN, account name, first or last name, or organization.
When the process is complete, you will be provided with a QR code and the secret to enter on your mobile device.DO NOT CLOSE THIS WINDOW UNTIL THR QR CODE HAS APPEARED.
After Setup/Using TOTP
After setup, when logging in, you will first be prompted to enter your PIC and the OTP displayed on your device together in one line. Then you will enter your password as usual.
